The service
You upload a blank copy of a paper form used by your practice. We rebuild it as a web app that your patients fill out and sign, and we deliver a first version within 24 hours of your confirmation. We build two forms a day within 24 hours: on a busier day, a form confirmed later gets 48 or 72 hours, and we show you that time before you confirm. These times run every day of the week, weekends included.
Proof options chosen at upload (photo uploads, photo at signature, location at signature) are built into your form at no extra cost.
Revisions
You review a preview of your form. Revisions are unlimited until you approve it. The delivery time applies to the first version only.
Free month and price
Your free month starts the day you approve the form and it goes live. At the end of the free month, the subscription for that form starts at $75 per month only if the form was used at least 10 times by patients during the free month. Test submissions do not count.
If the form was used fewer than 10 times, we pause it and charge nothing. You can reactivate it for $75 per month within 30 days. After 30 days without reactivation, the form is deleted. Before deleting it, we share its folder of patient records with your practice email. We never delete your patient records.
There is no setup fee. The price applies per form.
If we are late
If the first version is not delivered within the time we showed you, we add a second free month to that form's trial, automatically.
Your card
You save a card when you confirm an upload. Nothing is charged at that moment. Your card is charged only under the conditions above.
Your responsibilities
You upload only blank forms, with no patient information. You are responsible for the content of your forms and for keeping your patient records as required by the laws that apply to your practice.
