Why per form?

You pay for what patients actually use.

A practice with one intake form should not pay like a hospital. Each form has its own free month, its own count and its own price. Add a second form only when the first one earns its place.

Why the 10-use rule?

If it's not used, it's not worth $75.

We would rather pause a form than charge you for a tool your front desk ignores. Ten real patient submissions in a month is our line between "nice idea" and "part of your day".

If your first version misses the time we gave you, a second free month is added to that form. Automatically.

Barely used? Nothing charged.

Under 10 real uses in the free month, we pause the form and your card is never charged.

Paused? Your records stay yours.

We only ever pause or remove the form itself. Your patient records are never deleted.

Why do you need a card if the first month is free?

To hold your build spot. Your bank verifies the card and nothing is charged. From month two, it is charged only if the form was used at least 10 times.

What counts as a use?

One form completed and submitted by a real patient. Test submissions, including the ones you run while reviewing the preview, never count.

We have several forms. How does that work?

Each form has its own price, its own free month and its own count. You only pay for the forms your patients actually use.

What happens if our form gets paused?

You get an email with a button to reactivate it for $75 per month. You have 30 days. Your patient records are never touched.

Is there a setup fee?

No. Building your form from your paper is included in the price.
